Club Activity Report (pdf) Monthly by the 10th
Club Membership Report (online at the LCI website; username & password required) Monthly by the end of the month
Zone & Region Chair Visitation Report (pdf) After each Club Visit
PU101, Club Officer Report (pdf)  By April 15th of each year
Annual Sight Activity Report  (updated for 2006) By April 15th of each year
Annual Hearing Activity Report  (updated for 2006) By April 15th of each year
Annual Club Activity Report (pdf) By May 30th of each year

Please familiarize yourself with the contents of these reports as early as possible.  It will make things easier later on when the reports are due.
